Develop and implement individualized treatment plans based on patient needs. Provide medication management and monitor patient progress. Collaborate with interdisciplinary teams to ensure holistic patient care. Offer addiction counseling and support as needed. Maintain accurate and timely documentation in compliance with healthcare standards. Requirements PA License Enrolled in New York Medicare, Medicaid Department Experience in outpatient settings preferred.
