This is a part-time (4 days per week with benefits ) primary care medical director role providing patient care in a post-acute care setting at a skilled nursing facility (SNF) located in Rome, New York . You will round 4 days a week with the flexibility to choose your schedule.

Job Duties

• Primary care rounding in skilled nursing facilities
• Manage chronic disease, diagnose illnesses and medical conditions, and implement plans of care
• Collaborate with clinicians, facility leaders, and others regarding care and services
• Build relationships with patients, their families, and facility staff to facilitate effective treatment delivery
• Provide oversight, guidance, and mentorship to nurse practitioners
• Conduct clinical case reviews as needed and share tools, information, and guidelines related to quality of care
• Represent TeamHealth externally by engaging and establishing/maintaining relationships within the post-acute care community
• Collaborate with leadership to review care coordination initiatives, develop intervention plans, and share ideas about network management

Qualifications

• Minimum of 1 year primary care experience
• Post-acute care experience is a plus
• Active and unrestricted New York license or application in process
• Active and unrestricted DEA/prescriptive authority or application in process
