Now Hiring: Hospital-at-Home RN | Full-Time

📍 Madison, WI | High-Acuity, Patient-Centered Care

This full-time role supports a growing hospital-at-home program, providing acute care to patients in their homes. With full independence in your practice, you’ll manage complex cases, perform procedures, and coordinate care with interdisciplinary teams.

Key Responsibilities:

• Perform full-scope nursing assessments, interventions, and care coordination for acute patients at home
• Apply critical thinking to identify, prevent, and manage complex patient care issues
• Use biometric monitoring and respond to vital sign data
• Provide individualized patient education on disease management, medications, and treatments
• Perform procedural skills including IV therapy, phlebotomy, wound care, catheter care, and medication administration
• Collaborate with remote providers and interdisciplinary teams for seamless patient care
• Prepare and coordinate safe hospital-to-home transitions, leveraging community resources

Schedule:

• 40 hours/week | 10-hour shifts
• Weekend/holiday rotation
• Must reside in Madison, WI area

Qualifications:

• Active WI RN license (or eligible to obtain by start date)
• Associate’s/Bachelor’s Degree in Nursing
• 2+ years nursing experience; acute, ICU, ER, or home health preferred
• CPR/BLS certification
• Strong clinical judgment and high-acuity care experience
• Proficient with EMR systems and technology-enabled care
