Patient Care Coordinator - Health Coach:
- We are seeking a Patient Care Coordinator - LVN/LPN or RN to become a part of our team!
- This position would be 20 hours per week initially and full time within 90 days.
- You will coordinate patient care and coaching programs with remotely monitored patients.
- These programs focuses on patient care and compliance as well as diet and exercise.
- This is a 100% work from home position. Anywhere in the USA.
Responsibilities:
- Communicate care information from programs.
- Engage new patients enrolled.
- Help patients track and report changes in condition.
- Update medical record systems with condition changes.
- Assist patients with healthcare questions and concerns.
- Help patients achieve goals.
- Get patients additional care from their providers as needed.
Qualifications:
- Previous experience in healthcare or other medical fields.
- Experience in remote health coaching or patient management is an asset.
- Experience in a patient-facing role.
- Previous medical clinic and work from home experience is an asset.
- Current Nursing license.
- Strong interpersonal qualities.
- Strong organizational and time management ability.
- Strong technical skills.
- Excellent written and verbal communication skills.
- Understanding of medical record systems.
