Overview: The RN Care Manager is a critical member of the Care Management Team, collaborating with patients, caregivers, physicians, social workers, payers, and other healthcare professionals. This role is responsible for promoting effective communication, transitional care, and utilization management, all while respecting patient autonomy and addressing social determinants of health.
