Requirements:

• MD\nor DO degree from an accredited medical school.

• \n\nCompleted\nresidency in Family or Internal Medicine accredited by the Accreditation\nCouncil for Graduate Medical Education

• \n\nBoard\ncertified or board eligible in Family or Internal Medicine

• \n\nActive\nand unrestricted medical license to practice in Arizona.

• \n\nCurrent\nDEA registration for prescribing controlled substances.

• \n\nPrior\nexperience in a family medicine setting preferred.
